Netza,
On November 28, 2021, we issued an OWTS permit for the proposed residence
at 10991 County Road 320 near Rifle in Garfield County, Colorado (OWTS Permit #
SEPT-7069). As stated on the permit: “Contact the designer and Garfield County Public
Health at least 48 hours prior to requesting final inspection of the installed system prior
to backfill.” However, GCPH did not receive a request for a final inspection. On January
10, 2022, we received a letter from the designer, Michael Berry of Huddleston-Berry
Engineering & Testing, LLC stating that “based on our inspection, HBET certifies that
the OWTS was constructed in accordance with the design.” Upon receiving this letter,
we requested photos of the installation. Having not received a response from neither the
designer nor the contractor, we again asked for photo documentation of the system, so
that we could verify what was installed. After this second request, the contractor
submitted a video which showed the system, to the best of our knowledge. The as-built
drawing submitted by Mr. Berry indicates that a 1,000-gallon tank was installed followed
by the Soil Treatment Area (STA). The video submitted by the contractor shows a
distribution box with a baffle at the inlet, which distributes effluent to a four-row chamber
bed with 13 chambers per row (52 chambers total, 780 ft2). Additionally, the video also
shows inspection ports at the four corners of the bed, a poly septic tank (with risers
providing access to grade), and a cleanout between the house and the tank. We visited
the site on March 8, 2022 and verified the general location of the OWTS components.
At the time of our site visit, the risers for the septic tank and distribution box were not
visible. On March 15, the contractor emailed us photos showing the risers for the tank
and distribution box extending to grade, as well as an effluent filter in the outlet tee of
the tank, and finally grading over the STA that will promote positive drainage of surface
water runoff off of the STA. In our opinion, this system was installed in general
