Phase Con provided final clearance air monitoring services at the project
site listed above on October 29,202L Air samples were situated inside
the enclosure so as to be representative of the entire work space. Air
samples were obtained utilized aggressive sampling methods to aid in
ensuring that any fibrous material left in the enclosure would be
entrained in the air during the sampling period.
Abatement Description :
Abatement of ACM sheet vinyl flooring from the break room area.
Comments and Recommendations :
pCM analysis of the final clearance air samples revealed airborne fiber
levels inside the containment area(s) to be less than the Colorado State
Maximum Allowable Asbestos Level (MAAL) and the EPA clearance level

Phase Con provided limited asbestos building inspection services at theproperty described above on August s, zo21 was performed by Mr.Douglas close, a colorado state and EpA Asbestos auitoingInspector/Management Planner. The scope of work consisted of buildingmaterials as indicated by the client. Suspect materials within the scopeof work were sampled and analyzed as per Colorado State Regulation#8.
A. random sampling scheme was utilized in choosing sampling locations.This scheme used a grid system and randomly generated numbers. TheInspector, MF. Douglas close, classified the süspect materials by type(Thermal system Insulation (TSI), surfacing Materials, andMiscellaneous Materials), and obtained the appropriate number ofsamples of each material based upon material type, and quantity.
Tables are provided which detail the individual samples obtained, thela-l'oratory analysis results for these samples, the quantity and conditionof Asbestos containing Materiar (AcM); the rocations of eachhomogeneous area, and other materiat spätiric information. potentialfor damage to materiars information i; provided in the foìtowingsampling information tables.
The following materials were positive for regutated asöesfoscontent ()ro/o asöestos) as verified by point cıunt analysis:
' thget vinyl Flooring - present as the bottom layer of vinyl flooringin the break room/kitchen area.
Thís Asbestos containing Materiar (AcM) is regurated by the state ofcolorado and must be handred, removed and disposed oi oy a ricensedasbestos abatement contractor (General Abatement Certificate holder)if it will be dlsturbed in the construction work in quantities which exceedthe CDPHE trigger level of 160 square feet or the volume equivatent ofa 55 gallon drum.
I recommend that even smaller quantities (less than the trigger levels)
of this ACM be removed, damaged or disturbed only by trained, qualified
and properly protected personnel in the employ of a licensed asbestos
abatement contractor. Failure to use properly trained and licensed
personnel and appropriate engineering controls will likely result in the
spread of asbestos contamínation in the home and exposure to building
occupants creating a significant health hazard. If employees are used
for the handling or removal of this ACM all applicable OSHA regulations
must be adhered to under OSHA classifìcation of Class I asbestos work.
If any previously unknown or undiscovered materials are found during
demolition activities which are suspect for asbestos content, then these
materials should be tested for potential asbestos content prior to
continuing any further demolition work.
Disclaimer
The inspection services process consists of initial inspection/sampling
and sampling of subsequently discovered hidden or inaccessible building
materials as the inspection/a batement/demol¡tion process occurs within
the building. Discovery of hidden or inaccessible building materials by
any interested parties can and will occur at any time during the
inspection/abatement/demolition process and necessitates stopping
demolition until the discovered materials can be properly
ínspected/sampled. The discovery of hidden materials during the
building renovation/demolition process is common, should be expected
by the client and is a normal part of the inspection process.
This inspection was performed using non-invasive sampling techniques.
This report does not imply, or guarantee that every material on the
property, or in the property building, which may potentially have
asbestos as a component has been identified and/or sampled. A
guarantee that all asbestos materials have been identified and/or
sampled would require cost-prohibitive and destructive sampling
protocols. Furthermore, identified asbestos containing materials may
be in areas which are inaccessible or hidden due to their application
during the construction process and their subsequent enclosure or
covering with building and finish materials. Areas behind walls, inside
chases, or other hidden, covered or enclosed areas were not included
within the scope of this inspection, as destructive techniques would have
been required to access these areas. These areas should be inspected
whenever renovation or demolition activities are scheduled which may
disturb the materials within or beneath these barriers. 'Overspray' or
excess materials from the installation or application process of asbestos
containing materials is common, should be expected in the vicinity of
installed asbestos containing materials, and is often present either
covered by other building materials or in hidden or unexpected
locations. This inspection cannot determine the presence of these
materials and their locations and therefore the determination of these
locations and material quantities is outside of the scope of thls
inspection.
Bulk material samples are obtained in accordance with applicable
regulations, industry standard techniques and analyzed by a NVLAP
accredited laboratory; however, due to the asbestos content fluctuations
which may occur in a building material due to the application and/or
initial mixing process no guarantee can be made as to an 'exact'
percentage asbestos content (this includes'no asbestos detected' and
'trace' contents) which represents the entirety of the material (asbestos
content fluctuations can and will occur throughout a building material).
Abatement cost estimates and material quantity estimates are
approximate only (due to the hidden nature of many of the materials),
and are provided only as a general guideline to the client. More than
one licensed Colorado State asbestos abatement contractor should be
consulted to determine actual abatement costs of the ACBMs described
above. Actual material quantit¡es can only be determined by complete
removal of covering materials.

Analysis and Method
Summary of polarizing light m¡croscopy (PLM i Stereomicroscopy bulk asbeslos analysis) using the melhods described in 40CFR parl 763
Appendix E to Subpart E (lnterim and EPA 600 / R-93 / 1 16 (lmproved). The sample is fkst viewed with the aid of stereomicroscopy. Numerous
liquld slide pr€parations are created for analysis under the polarized microscope where identifications and quantifications are preformed. Calibrated
liquid refractive oils are used as liquid mouting medlum. These olls are used for identification (dispersion staining). A calibrated visual estimalion is
reported, should any asbestiform mineral be present. Olher techniques such as acid washing are used in conjugation with refractive oils for
detection of smaller quantit¡es of asbeslos. All asbeslos percentages are based on calibrated visual estimation traceable lo NIST standards for
regulated of asbeslos. Traceab¡l¡ty to measuremenl and calibration is achieved by using known amounts and types of asbestos from slandards
where analyst and laboratory accuracy are measured. As little as 0.001o/o asbestos can be detected in favorable samples, while deteclion in
unfavorable samples may approach the detection limit of 0.50% (well above the laboratory definition of trace).
Discussion
Vermiculite contain¡ng samples may have trace amounls of act¡nol¡te-tremolite, where not found be PLM should be analyzed using TEM methods
and / or water separat¡on techniques. Suspected actinolite/vermicul¡te presence will be indicated through the sample comment seclion of this reporl.
Fibrous talc conlain¡ng samples may even contain a related asbestos fiber known as anthophyllite. Under certain conditions the same fiber may
actually contain both talc and anthophyllite (a phenomenon called intergrowth). Again, TEM detection methods are recommended. CA Labs pLM
report comments will denote suspected amounts of asbesliform anthophyllite with talc, where further analysis is recommended.
some samples (floor t¡les, surfacings, etc.) may conta¡n Tibers too small to be detectable by PLM analysis and should be analyzed by TEM bulk
protocols.
A "trace asbeotos" will be reportcd if the analyst observes far less than 1 o/o asbeslos. CA Labs defines "lrace asbestos. as a few flbers delected
by the analyst in several preparat¡ons and will indicate as such under these circumstances.
Quantilication of <1% will actually be reported as <=17o (allowable variance close to 1ol" is high). Such results are ideal for point counling, and the
technique ¡s mandatory for friable samples (NESHAP, Nov. 1990 and clarification letter B May 1991) under 1% percent asbestos and the ,'trace
asbestos". ln order to make all ínitial PLM reports issued from CA Lâbs NESHAP compl¡anl, all <1olo asbeslos results (except floor t¡les) w¡ll
be point countêd at no add¡tional charge.

CA Labs is accredited by lhe Naliona¡ Voluntary Accreditat¡on Program (NVLAP) for selected test methods for airborne fiber analysis (TEM), andfor bulk asbeslos tiber analysis (PLM).
