Onsite Wastewater Treatment System (OWTS) Installation Observations

ALL SERVICE septic, LLC observed the installation of the onsite wastewater treatment system (OWTS)
on September 20, 2021 for the subject property. You, the property owner, installed the system.
The OWTS design is based on 4-bedrooms. An average daily wastewater flow of 525 GPD was
used.
The system installation included a 1500-gallon, two-compartment concrete septic tank with an Orenco®
Pro Pak and PF3005 pump in the second compartment of the septic tank. Floats were set to dose
approximately 70 gallons per pump cycle. You, the property owner, performed start-up of the pumping
system.
The control panel for the pumping system is located within line of sight of the septic tank.
Effluent is pressure dosed through a 1.5-inch diameter pump line to a Model 6402A Automatic Distributing
Valve (ADV) which was located at the high point and accessible from grade. The ADV has two 1.5-inch
diameter transport lines to two separate gravelless chamber beds.
Effluent is pressure dosed through a 1.5-inch diameter distribution lines to two 6.5’ x 51’ beds. A minimum of
3-feet of sand filter material was installed in the over-excavated footprint. There was at least 6-feet of
undisturbed soil between each bed. Sand filter material must be clean, coarse sand, all passing a screen
having four meshes to the inch. The sand must have an effective size between 0.15 and 0.60 mm. The
uniformity coefficient must be 7.0 or less. Material meeting ASTM 33, for concrete sand, with three percent
or less fines passing 200 mesh sieve may be used. A gradation of the sand media must be submitted
to this office prior to obtaining the sand.
Laterals were 1.5-inches in diameter with 5/32-inch diameter orifices facing down, spaced 3-feet on
center, installed on the GeoMat™ with the filter fabric over the laterals. Laterals were placed 2-feet from the
edges of the bed, with 2.5-feet between the laterals. Each 1.5-inch diameter lateral ends in a sweeping ell
facing up with a ball valve for flushing. Laterals were covered by the GeoMat™ soil separation fabric and
at least 1-foot of topsoil. Inspection ports were placed at each corner of each bed. Ports may be cut to
grade and covered with a valve box for access.
The OWTS was generally installed according to specifications. This observation is not a guarantee of
workmanship and/or parts and materials. ALL SERVICE septic, LLC should be notified if changes are
made to the OWTS in the future. Any additional OWTS construction must be according to the county
regulations.
LIMITS:
Observations are limited to components that are visible at the time of the inspection. The installer must
have documented and demonstrated knowledge of the requirements and regulations of the county in
which they are working. The quality of the installation is dependent of the expertise of the installer, soil
type, and weather conditions.

Permit Conditions:
This OWTS Permit is approved on the condition of compliance with the revised report and design as
submitted by All Service Septic dated July 1, 2020 based on Soil Type R-0. Changes to the design and
permit must be approved by Garfield County Public Health and the designer prior to construction.
The new OWTS will serve a four-bedroom residence and consist of a 1500 gallon septic tank with a
pump in the second compartment set level and accessible at grade followed by an automatic
distributing valve set level and to grade to distribute effluent to 663 sq. ft. of STA. The STA consists
of two separate 6.5' x 51' pressure -dosed GeoMat beds with at least 6 -feet of undisturbed soil
between each bed. A minimum of 3 -feet of sand filter material will be installed using secondary sand
media. Laterals will be spaced 2 feet from the edges of the bed and 2.5 -feet apart with flushing
valves at the end of each lateral and inspection ports at each corner of the bed. Maintain all setbacks
and elevations as designed.
Contact the designer and Garfield County Public health at least 48 hours prior to requesting final
inspection of the installed system prior to backfill.
Agreement
All installations must comply with all requirements of the Garfield County OWTS regulations
including but not limited to the conditions specified above.
In consideration of the issuance of the permit, the applicant hereby agrees to comply with all
laws and regulations related to the zoning, location, construction and erection of the proposed
OWTS and associated structures for which this permit is granted. The applicant further agrees
that if the above said regulations are not fully complied with in the zoning, location, erection
and construction of the above described OWTS and structure, the permit may then be revoked
by notice from the Health Department and immediately become null and void.
The issuance of a permit and specifications of terms and conditions therein will not constitute
assumption of liability, nor create presumption that the local public health agency or its
employees may be liable for the failure or malfunctioning of any system. Permit issuance will
not constitute a certification that the system, the equipment used in the system, or any
component used for system operation will ensure continuous compliance with the provision of
the OWTS Act, the regulations adopted thereunder, or any terms and conditions of a permit.
Permit Approval
Plans and specifications for the proposed OWTS have been reviewed and are considered
satisfactory. Permission is hereby given to the property owner(s) to perform the work indicated
in accordance with the Garfield County OWTS Regulation. This Permit will expire 1 year from
the date of issuance unless construction on the system has commenced. An "as -built" drawing
must be submitted and inspection of the system must be completed by GCPH before final
approval of the system will be issued.
