Finishing Touch Dirtworx (Jesse Schoeller) installed the system. The OWTS design is based on 4-bedrooms. An average daily wastewater flow of 525 GPD was used. The existing soil treatment area (STA) was abandoned in place. The existing sewer line from the house to the septic tank remained. The sewer line entering the existing 1250-gallon, two compartment concrete septic tank was properly aligned. An Orenco® Biotube Effluent Filter was installed in the second compartment, on the outlet tee in the septic tank. This filter should be cleaned annually, or as needed. A new 4-inch SDR-35 sewer line was installed to a new distribution box. The distribution box was accessible from grade with flow equalizers installed on each outlet pipe in the distribution box to assure equal flow to each trench. Five gravelless chamber trenches were installed. Four trenches consisted of 16 ‘ADS’ ARC 36 Low Profile chambers and one trench with 10 ‘ADS’ ARC Low Profile chambers (to avoid removing mature trees) for a total of 74 chambers and 1110 square feet of infiltrative area. There was at least 4-feet of undisturbed soil between each trench. Inspection ports were installed at the beginning and end of each trench. Ports may be cut to grade and placed in sprinkler boxes for access. Design Calculations: Average Design Flow = 75 GPD x 2 people/bedroom x 3 Bedrooms + 75 GPD (4th Bedroom) = 525 GPD LTAR = 0.35 GPD/SF 525 GPD / 0.35 GPD/SF x 1.0 (gravity trenches) x 0.7 (chambers) = Min. 1050 SF 74 chambers installed x 15 SF per chamber = total 1110 SF The OWTS was generally installed according to specifications. This observation is not a guarantee of workmanship and/or parts and materials. CBO Inc. should be notified if changes are made to the OWTS in the future. Any additional OWTS construction must be according to the county regulations. LIMITS: Observations are limited to components that are visible at the time of the inspection. The installer must have documented and demonstrated knowledge of the requirements and regulations of the county in which they are working. The new OWTS will serve an existing 4 bedroom residence.The system will consist of an existing 1,250 gallon septic tank with a new effluent filter installed in the outlet tee of the tank.Effluent will gravity flow to a distribution box set level with access to grade to distribute effluent to 1,080 ft2 of new STA.A Bull Run valve may be installed to allow alternate flow between the existing,over saturated STA once dried out)and the new STA.The new STA will consist of five chamber trenches. Each trench will consist of 18 chambers per trench 90 Quick4 chambers total).Inspection ports are to be installed at each end of each trench.Maintain all setbacks and elevations as designed. Contact the designer and Garfield County Public Health at least 48 hours prior to requesting final inspection of the installed system prior to backfill. Agreement All installations must comply with all requirements of the Garfield County OWTS regulations including but not limited to the conditions specified above. In consideration of the issuance of the permit,the applicant hereby agrees to comply with all laws and regulations related to the zoning,location,construction and erection of the proposed OWTS and associated structures for which this permit is granted.The applicant further agrees that if the above said regulations are not fully complied with in the zoning,location,erection and construction of the above described OWTS and structure,the permit may then be revoked by notice from the Health Department and immediately become null and void. The issuance of a permit and specifications of terms and conditions therein will not constitute assumption of liability,nor create presumption that the local public health agency or its employees may be liable for the failure or malfunctioning of any system.Permit issuance will not constitute a certification that the system,the equipment used in the system,or any component used for system operation will ensure continuous compliance with the provision of the OWTS Act,the regulations adopted thereunder,or any terms and conditions of a permit.
