Onsite Wastewater Treatment System (OWTS) Installation Observations

Michael and Tammy,
CBO Inc. observed the installation of the onsite wastewater treatment system (OWTS) on May 12, 2022
for the subject property. You, the property owner, installed the system.
A 5-bedroom, single-family residence presently exists and was recently remodeled and is utilizing an
existing OWTS. The OWTS is documented by Garfield County Individual Sewage Disposal System
Permit 1845 which received final approval on November 16, 1990. The system was originally sized to
accommodate 3-bedrooms. The OWTS was inspected by Altitude Septic and documented in a report
dated November 30, 2021. As a result of the inspection and the increased bedroom count in the
residence, modifications to the OWTS were required.
The existing septic tank was abandoned. A new, 1500-gallon, two-compartment Infiltrator® poly septic
tank with an Orenco Biotube Effluent filter on the outlet tee was installed. This filter must be cleaned
annually, or as needed.
The OWTS was generally installed according to specifications. This observation is not a guarantee of
workmanship and/or parts and materials. CBO Inc. should be notified if changes are made to the OWTS
in the future. Any additional OWTS construction must be according to the county regulations.
LIMITS: Observations are limited to components that are visible at the time of the inspection. The installer must
have documented and demonstrated knowledge of the requirements and regulations of the county in
which they are working. The quality of the installation is dependent on the expertise of the installer, soil
type, and weather conditions.

Michael and Tammy,
On May 11, 2022, we visited the site of the Koski Residence at 605 Lions Ridge Rd near
Carbondale in Garfield County, Colorado to observe the OWTS tank only installation pursuant to
OWTS Permit # SEPT-7334. At the time of our site visit, the owner had installed the new sewer
line and cleanout adjacent to the residence, the new poly 1,500-gallon two-compartment septic
tank. The septic tank was installed level with access brought to grade. An effluent filter had
been installed in the outlet tee of the septic tank. Photos were later sent showing the installed
risers and an extension on the effluent filter handle. In our opinion, this system was installed in
general conformance with Garfield County OWTS Regulations. Please submit an As-built
drawing of the system when available. No further action or review from Garfield County
Environmental Health is required regarding this permit.
