HomeMy WebLinkAboutFinal GCPH Acceptance LetterGarfield County Public Health Department – working to promote health and prevent disease   Public Health Frank Hart PO Box 12310 Columbia, SC 29211 Via email: fhart@sc.rr.com Carla.ostberg@gmail.com Subject: Final Acceptance Letter 1024 County Road 112 Carbondale, CO 81623 OWTS Permit #: SEPT-7144 February 17, 2023 Frank, On behalf of Garfield County Public Health, Carla Ostberg of CBO, Inc. visited the site at 1024 County Road 112 near Carbondale in Garfield County, Colorado on October 19, 2021 to observe the OWTS installation pursuant to OWTS Permit # SEPT-7144. At the time of the site visit, the contractor had installed the new effluent line, the distribution box, and the five chamber trenches. The distribution box was installed level with access brought to grade. An effluent filter had been installed in the outlet tee of the existing septic tank. The chamber trenches consisted of 15 Quick4 chambers per trench, for a total of 75 chambers (900 ft2). The trenches were level, with at least four feet between each trench. The bed consisted of clean, washed gravel as specified in the design. Additionally, inspection ports were present at each end of each trench. In our opinion, this system was installed in general conformance with Garfield County OWTS Regulations. No further action or review from Garfield County Environmental Health is required regarding this permit. Thank you, Edward R. “Ted” White, P.E.
