Onsite Wastewater Treatment System (OWTS) Installation Observations

CBO Inc. observed the installation of the onsite wastewater treatment system (OWTS) on October 17,
December 7, 2022 and February 14, 2023 for the subject property. Avalanche Excavation installed the
system.
An existing well on the property serves both residences potable water. The well was verified to be at
least 50-feet from both septic tanks and at least 100-feet from the soil treatment area (STA).
The new OWTS design was based on 3-bedrooms in the Main Residence and 2-bedrooms in the
Accessory Dwelling Unit (ADU). An average daily wastewater flow of 750 GPD was used.
The existing septic tank and STA serving the main house remains in use at this time. The main
house may connect to the new OWTS in the future. At that time, the existing 4-inch diameter sewer line
serving the main house will be replaced. The sewer line must be encased (See Encasement Detail
W3.0/4 of the OWTS Design Packet dated August 20, 2022) where it comes within 50-feet of the well.
There must also be a cleanout a minimum of every 100-feet. At the time the main house is connected to
the new OWTS, the existing septic tank and STA must be properly abandoned. Documentation of this
work must be provided to this office and Garfield County Environmental Health Department.
The 4-inch diameter SDR-35 sewer line exiting the ADU has a double-sweep clean out and a minimum
2% grade to the septic tank.
The system installation included a 1500-gallon, two-compartment Infiltrator® poly septic tank with an
effluent filter on the outlet tee, followed by a 500-gallon, single-compartment Infiltrator® poly pump
chamber with an Orenco® Biotube Pump Vault and an Orenco® PF3005 pump. The floats were set to
dose approximately 85 gallons each pump cycle. Both the septic tank and pump chambers have
effluent filters that must be cleaned annually, or as needed. Valley Precast out of Buena Vista
performed start-up of the pumping system. The control panel is located within line of sight of the pump
chamber.
Effluent is pumped through a 1.5-inch diameter pump line to an Orenco® automatic distributing valve
(ADV), model 6403. The ADV was placed at a high point in the system in an insulated riser with access from
grade.
Effluent is pressure dosed through 1.5-inch diameter distribution lines to three beds, each 7’ x 44.5’. The
over-excavation extended approximately 4-feet below grade in all beds and no partial mounding was
necessary. A minimum of 3-feet of sand filter material was installed in the over-excavated footprint. A
gradation of the sand media is enclosed.

Laterals are 1.5-inches in diameter with 5/32-inch diameter orifices facing down, spaced 3-feet on center,
installed on the center of each GeoMat™ with the filter fabric over the laterals. Two 39” wide GeoMat™
sheets were placed on the 7-foot wide beds.
Each 1.5-inch diameter lateral ends in a sweeping ell facing up with a ball valve for flushing. Valves were
placed in a 10-inch sprinkler box for access. Laterals were covered by the GeoMat™ soil separation fabric
and at least 1-foot of topsoil. Inspection ports were placed at each corner of each bed. Ports may be cut
to grade and covered with a valve box for access.
The OWTS was generally installed according to specifications. This observation is not a guarantee of
workmanship and/or parts and materials. CBO Inc. should be notified if changes are made to the OWTS
in the future. Any additional OWTS construction must be according to the county regulations.
LIMITS: Observations are limited to components that are visible at the time of the inspection. The installer must
have documented and demonstrated knowledge of the requirements and regulations of the county in
which they are working. The quality of the installation is dependent on the expertise of the installer, soil
type, and weather conditions.

Permit Conditions:
This OWTS Permit is approved on the condition of compliance with the design as submitted by CBO
Septic Consulting dated February 3, 2022. Changes must be approved by Garfield County Public
Health and the designer prior to construction.
The new OWTS will serve a new 2-bedroom Accessory Dwelling Unit (ADU). The system will consist
of a new, two-compartment minimum 1,000-gallon septic tank set level with access to grade and a
high-head pump in the second compartment of the tank. Effluent will be pressure-dosed to an
automatic distributing valve (ADV) set level with access to grade at the high point of the system that
will distribute effluent to 437.5 ft2 of STA. The new STA will consist of two unlined sand filter beds
constructed of 3 feet of secondary sand material. The beds will be partially mounded, with 2.5 feet
below grade and six inches above grade. The pump and effluent lines will be constructed of 1.5-inch
diameter Schedule 40 PVC. The laterals in each bed will be connected with level manifolds, and the
laterals shall have 5/32-inch diameter orifices facing down spaced every 2 feet on-center (O.C.),
installed on GeoMat placed directly on the sand. Inspection ports are to be installed at each corner
of each bed. Maintain all setbacks and elevations as designed.
Contact the designer and Garfield County Public Health at least 48 hours prior to requesting final
inspection of the installed system prior to backfill.
Agreement
All installations must comply with all requirements of the Garfield County OWTS regulations
including but not limited to the conditions specified above.
In consideration of the issuance of the permit, the applicant hereby agrees to comply with all
laws and regulations related to the zoning, location, construction and erection of the proposed
OWTS and associated structures for which this permit is granted. The applicant further agrees
that if the above said regulations are not fully complied with in the zoning, location, erection
and construction of the above described OWTS and structure, the permit may then be revoked
by notice from the Health Department and immediately become null and void.
The issuance of a permit and specifications of terms and conditions therein will not constitute
assumption of liability, nor create presumption that the local public health agency or its
employees may be liable for the failure or malfunctioning of any system. Permit issuance will
not constitute a certification that the system, the equipment used in the system, or any
component used for system operation will ensure continuous compliance with the provision of
the OWTS Act, the regulations adopted thereunder, or any terms and conditions of a permit.
Permit Approval
Plans and specifications for the proposed OWTS have been reviewed and are considered
satisfactory. Permission is hereby given to the property owner(s) to perform the work indicated
in accordance with the Garfield County OWTS Regulation. This Permit will expire 1 year from
the date of issuance unless construction on the system has commenced. An "as-built" drawing
must be submitted and inspection of the system must be completed by GCPH before final
approval of the system will be issued.
