HomeMy WebLinkAboutGCPH Final acceptance Letter 03.24.2023Garfield County Public Health Department – working to promote health and prevent disease   Public Health Matt and Melissa Goldstein 503 West Castle Court Glenwood Springs, CO 81601 Via email: mlggws@yahoo.com  chris@mountaincross-eng.com Subject: Final Acceptance Letter Goldstein Residence 620 Juniper Lane Carbondale, CO 81623 OWTS Permit #: SEPT-7128 March 24, 2023 Matt and Melissa, On February 2, 2023, we visited the site of the Goldstein Residence at 620 Juniper Lane near Carbondale in Garfield County, Colorado to observe the OWTS installation pursuant to OWTS Permit # SEPT-7128. At the time of our site visit, the contractor had installed the new sewer line and cleanout adjacent to the residence, the new poly 1,500-gallon two-compartment septic tank, the 500-gallon dosing tank with a dosing siphon in the dosing tank, the new effluent line, the distribution box, and the four gravelless chamber trenches. The septic tank and distribution box were installed level with access brought to grade. An effluent filter had been installed in the outlet tee of the septic tank. The trenches were level, with 4-feet between the trenches. The trenches consisted of 16 ARC36 chambers per trench (64 total chambers, 960 ft2 total STA). Additionally, inspection ports were present at each end of each trench. We received an installation certification letter and as-built drawing from the design engineer on March 17, 2023. In our opinion, this system was installed in general conformance with Garfield County OWTS Regulations. Thank you, Edward R. “Ted” White, P.E.
