Ryan Ivy installed the system. The OWTS design is based on 4-bedrooms. An average daily wastewater flow of 525 GPD was used. The 4-inch diameter SDR-35 sewer line exiting the residence has a double-sweep clean out and a minimum 2% grade to the septic tank. The installation included a 1500-gallon, two-compartment Valley Precast concrete septic tank with an Orenco® Biotube Pump Vault and an Orenco® PF3005 pump in the second compartment of the septic tank. The floats were set to dose approximately 50 gallons each pump cycle. There is a filter in the second compartment of the septic tank that must be cleaned annually, or as needed. Risers were installed on the septic tank to bring access to grade. The control panel is located within line of sight of the septic tank. Valley Precast out of Buena Vista performed start up on the pumping system (see enclosed start-up letter dated December 7, 2021). They installed the pumping system and control panel after our visit to the property; therefore, I have approximated the control panel location on our drawing. A 1.5-inch diameter Schedule 40 pump line was installed from the pump to an Orenco® automatic distributing valve (ADV), model 6605. The ADV was placed at a high point in the system in an insulated riser with access from grade. Effluent is pressure dosed through 1.5-inch diameter pipes to five trenches, each with 8 ‘ADS’ ARC chambers, for a total of 40 chambers and 600 square feet of infiltrative area. There was at least 4-feet of undisturbed soil between each trench. Effluent is pressure dosed to 1.5-inch diameter laterals, which must be hung with zip ties from the underside of the chambers. Laterals will have 5/32-inch diameter orifices facing up, with the exception of the first and last holes facing down for drainage. Orifices were drilled 2-feet on center. Each lateral ends in a 90-degree ell facing up with a ball valve for flushing. Valves were placed in a valve box, accessible from grade, for access. Inspection ports were placed at the beginning and end of each trench. Ports may be cut to grade and covered with a valve box for access. The OWTS was generally installed according to specifications. This observation is not a guarantee of workmanship and/or parts and materials. CBO Inc. should be notified if changes are made to the OWTS in the future. Any additional OWTS construction must be according to the county regulations. 3-29-2023 LIMITS: Observations are limited to components that are visible at the time of the inspection. The installer must have documented and demonstrated knowledge of the requirements and regulations of the county in which they are working. Changes to the design and permit must be approved by Garfield County Public Health and the designer prior to construction. The new OWTS will serve a new four -bedroom residence. The system will consist of a new, two - compartment precast concrete 1,550-gallon septic tank set level with access to grade and a pump in the second compartment. A control panel for the pump will be placed within sight of the pump. Effluent will be pumped to an automatic distributing valve set level with access to grade distributing effluent to 600 ft2 of STA. The STA will consist of five trenches, each with 10 `Quick 4' chambers for a total of 50 chambers. Inspection ports are to be installed at the beginning and end of each trench. Maintain all setbacks and elevations as designed. Contact the designer and Garfield County Public Health at least 48 hours prior to requesting final inspection of the installed system prior to backfill. Agreement All installations must comply with all requirements of the Garfield County OWTS regulations including but not limited to the conditions specified above. In consideration of the issuance of the permit, the applicant hereby agrees to comply with all laws and regulations related to the zoning, location, construction and erection of the proposed OWTS and associated structures for which this permit is granted. The applicant further agrees that if the above said regulations are not fully complied with in the zoning, location, erection and construction of the above described OWTS and structure, the permit may then be revoked by notice from the Health Department and immediately become null and void. The issuance of a permit and specifications of terms and conditions therein will not constitute assumption of liability, nor create presumption that the local public health agency or its employees may be liable for the failure or malfunctioning of any system. Permit issuance will not constitute a certification that the system, the equipment used in the system, or any component used for system operation will ensure continuous compliance with the provision of the OWTS Act, the regulations adopted thereunder, or any terms and conditions of a permit. Permit Approval Plans and specifications for the proposed OWTS have been reviewed and are considered satisfactory. Permission is hereby given to the property owner(s) to perform the work indicated in accordance with the Garfield County OWTS Regulation. This Permit will expire 1 year from the date of issuance unless construction on the system has commenced. An "as -built" drawing must be submitted and inspection of the system must be completed by GCPH before final approval of the system will be issued. Box 739 Snowmass, CO 81654 Via email: harry@ski.com carla.ostberg@gmail.com Subject: Final Acceptance Letter Lot 8, Coulter Creek Ranch Carbondale, CO 81623 OWTS Permit #: SEPT-6509 December 9, 2021 Harry and Kathryn, On June 1, 2021, we visited the residence at Lot 8 of Coulter Creek Ranch near Carbondale in Garfield County, Colorado to observe the Onsite Wastewater Treatment System (OWTS) installation pursuant to OWTS Permit # SEPT-6509. The contractor had installed the 1,500-gallon septic tank, high-head pump, effluent lines, automatic distributing valve, and five gravelless chamber trenches as designed by CBO, Inc. Cleanouts were installed as required along the sewer line between the house and the septic tank. Risers were installed on the septic tank and automatic distributing valve, as required. Effluent lines and laterals consisted of 1.5-inch diameter Schedule 40 PVC. Laterals were zip-tied to the top of the chambers, with 5/32-inch diameter orifices every three-feet on-center. The first and last orifices on the laterals pointed down to allow for drainage. All other orifices pointed up. Each trench consisted of 8 ARC 36 chambers per trench (40 chambers total, 600 ft2 STA). The effluent line between the tank and the ADV was not installed at the time of our first site visit. On December 8, 2021, we received a letter from the contractor that indicated the effluent pump for the system had been installed, wired, and tested. In our opinion, this system was installed in general conformance with Garfield County OWTS Regulations. No further action or review from Garfield County Environmental Health is required regarding this permit. Thank you, Edward R. “Ted” White, P.E.
