March 22, 2023 310 Market Street Basalt, CO 81621 ATTN: Mr. Rafael Medina RE: Voss Residence Permit #BLRE-0602207625 1065 Hidden Valley Drive Glenwood Springs, CO. *Field Inspection Dear Mr. Medina, As requested, special inspection was conducted per the IBC 2018 Edition - Chapter 17: Structural Tests and Special Inspection and the Garfield County requirements. I arrived on site at 9:45 AM. I met with you to review project requirements and material ready for inspection. While on site I received and reviewed a copy of an email clarification from the project engineer of record Mr. Nate Decker, PE, SE with DB Structural Design accepting the 5/8" diameter by 12" deep all threaded rod utilizing Simpson Strong-Tie SET-XP Epoxy for a total of 10 each hold downs ties. Please refer to the attached email correspondence for project documentation. Continuous epoxy anchor inspection was conducted at the Foundation Plan Level for this project. I observed the anchor installed for the Holdown plates at grid line A & 5, A & 6, B & 6, B & 7, C & 1, D & 1, D & 7, and E & 7 as referenced on supplied engineering approved drawings provided by DB Structural design. A total of 10 all threaded anchors were installed at depths of 12" minimum as referenced on above drawings. Epoxy utilized for this project was Simpson Strong-Tie SET-XP Epoxy. I observed the drilled hole depth check, the cleaning of each drilled hole with a wire brush (3 times) and compressed air blown into the bottom of each hole, and the epoxy introduction and counterclockwise rotation and insertion of all the threaded anchors as per the manufacturer's instructions. Upon completion, I observed all epoxy anchors to be in compliance with project specifications and epoxy manufacturers recommendations.
