To Whom It May Concern:
I have and cornpleted a final review of the job notes, pier logs, and installation photos, on
912912023 for the following:
1. Supportworks - Model 288 Push Piers
r Push piers were observed reasonably level and vertical.o 5 Push Piers were added to the project scope due to unexpected site
conditions and to provide proper lift of the structure.. 3" to 3-l/4" Iift was achieved after installation of piers
o Final Inst¿llation pressure 4200 psi.
o Final installation depths ranged from 29'-8" to 50'-6"
Installation locations shown on Drawing #PRJ23025
One pier was dropped from the scope of the project after excavation revealed unexpected
site conditions.
The Supportworks equipment has been installed generally as designed to provide
additional stability to the structure. A lift was conducted and has been accepted by
the engineer.
The Push Piers were installed generally in accordance with the Repair Plan and
changes to the original plan made in the field. This is considered âcceptable.
This final inspection letter is limited to observations made from evidence obtained during a
project documents review. No destructive or invasive testing was performed. The final

warranty is irnplied.
