HomeMy WebLinkAboutGCPH Final Acceptance Letter 11.01.23Garfield County Public Health Department – working to promote health and prevent disease   Public Health Arvid and Janet Johnson 471 County Road 112 Carbondale, CO 81650 Via email: carrie@dzremodeling.com    carla.ostberg@gmail.com  Subject: Final Acceptance Letter 471 County Road 112 Carbondale, CO 81623 OWTS Permit #: SEPT-8187 November 1, 2023 On September 7, 2023, we visited the site of the residence at 471 County Road 112 near Carbondale in Garfield County, Colorado to observe the OWTS installation pursuant to OWTS Permit # SEPT-8187. At the time of our site visit, the owner had installed the new sewer line and cleanout adjacent to the residence, the new poly 1,500-gallon two-compartment septic tank with an effluent filter in the outlet tee of the tank, the new effluent line, the new distribution box, and the five gravelless chamber trenches. The septic tank was installed level with access brought to grade. The trenches consisted of 20 Infiltrator Quick4 chambers per trench (100 total chambers, 1,200 ft2 total STA). Additionally, inspection ports were present at each end of each trench. We received an as-built drawing of the constructed system and a letter certifying that the system was constructed in compliance with the permitted design from the OWTS designer dated September 27, 2023. In our opinion, this system was installed in general conformance with Garfield County OWTS Regulations. No further action or review from Garfield County Environmental Health is required regarding this permit. Thank you, Edward R. “Ted” White, P.E.
