Onsite Wastewater Treatment System (OWTS) Installation Observations

CBO Inc. observed the installation of the onsite wastewater treatment system (OWTS) on October 26,
2021 and April 22, 2024 for the subject property. RWI (Ryan Ivy) installed the system with the exception
of the sewer line from the house to the septic tank and the top two risers on the septic tank which were
installed at a later date when final grading was established.
The OWTS design is based on 3-bedrooms in the main house and 1-bedroom in a future detached
Accessory Dwelling Unit (ADU). An average daily wastewater flow of 600 GPD was used.
The ADU had not been constructed at the time of our inspection and the location/elevation of the OWTS
components accommodated only the main residence. The sewer line exiting the residence has a double-
sweep clean out.
The system installation included a 1250-gallon, two-compartment Forterra concrete septic tank with an
effluent filter on the outlet tee. Risers bring the manhole lids to grade for access. An extension handle
was installed on the effluent filter for ease of removal.
Effluent gravity flows to a distribution box, and then to five gravelless chamber trenches. The distribution
box was accessible from grade with flow equalizers installed on each outlet pipe in the distribution box to
assure equal flow to each trench.
Each trench consists of 12 ‘ADS’ ARC 36 Low Profile chambers for a total of 60 chambers and 900
square feet of infiltrative area. There was at least 4-feet of undisturbed soil between each trench.
Chambers were installed below the topsoil horizon. Inspection ports were installed at the beginning and
end of each trench. Ports were cut to grade and placed in sprinkler boxes for access.
The OWTS was generally installed according to specifications. This observation is not a guarantee of
workmanship and/or parts and materials. CBO Inc. should be notified if changes are made to the OWTS
in the future. Any additional OWTS construction must be according to the county regulations.
Items addressed after our April 22, 2024 visit included the following (as described by contractor):
• Extension rings were excavated/removed and two holes were grouted w/ Hydraulic cement on
both sides of each ring including the previously installed gasketed rings.
• Black bitumen like cordage/gasket was introduced along the inside geometry of rings.
• Old effluent filter was sawed off and a new 4" Full size BioTube filter was installed in its place.
• Excavated area was backfilled.
LIMITS: Observations are limited to components that are visible at the time of the inspection. The installer must
have documented and demonstrated knowledge of the requirements and regulations of the county in
which they are working. The quality of the installation is dependent on the expertise of the installer, soil
type, and weather conditions.

Permit Conditions:
This OWTS Permit is approved on the condition of compliance with the report and design as
submitted by CBO, inc. dated October 17, 2020. Changes to the design and permit must be approved
by Garfield County Public Health and the designer prior to construction.
The new OWTS will serve a new three -bedroom residence and detached studio. The sewer line from
the main residence will tie into the sewer line serving the studio with a "wye" fitting and additional
cleanout at the point where the sewer lines tie together. A new, two -compartment precast concrete
1250-gallon septic tank will be set level with access to grade with an effluent filter on the outlet tee.
Effluent will gravity flow to a distribution box set level with access to grade distributing effluent to
840 ft2 of STA. The STA will consist of five trenches, each with 14 'Quick 4' Standard Plus Infiltrator
chambers for a total of 70 chambers. Inspection ports are to be installed at the beginning and end of
each trench.
Maintain all setbacks and elevations as designed. The proposed pond must be lined to meet
minimum setback distances with current placement.
Due to potential for compaction of soil in the STA, Garfield County Public Health is adding a
condition of permit approval that staff must view an excavated trench prior to system install.
Contact the designer and Garfield County Public Health at least 48 hours prior to requesting soil
inspection and final inspection of the installed system prior to backfill.
Agreement
All installations must comply with all requirements of the Garfield County OWTS regulations
including but not limited to the conditions specified above.
In consideration of the issuance of the permit, the applicant hereby agrees to comply with all
laws and regulations related to the zoning, location, construction and erection of the proposed
OWTS and associated structures for which this permit is granted. The applicant further agrees
that if the above said regulations are not fully complied with in the zoning, location, erection
and construction of the above described OWTS and structure, the permit may then be revoked
by notice from the Health Department and immediately become null and void.
The issuance of a permit and specifications of terms and conditions therein will not constitute
assumption of liability, nor create presumption that the local public health agency or its
employees may be liable for the failure or malfunctioning of any system. Permit issuance will
not constitute a certification that the system, the equipment used in the system, or any
component used for system operation will ensure continuous compliance with the provision of
the OWTS Act, the regulations adopted thereunder, or any terms and conditions of a permit.
Permit Approval
Plans and specifications for the proposed OWTS have been reviewed and are considered
satisfactory. Permission is hereby given to the property owners} to perform the work indicated
in accordance with the Garfield County OWTS Regulation. This Permit will expire 1 year from
the date of issuance unless construction on the system has commenced. An "as -built" drawing
must be submitted and inspection of the system must be completed by GCPH before final
approval of the system will be issued.
