HomeMy WebLinkAboutGCPH Final acceptance Letter SEPT-7619Garfield County Public Health Department – working to promote health and prevent disease Public Health Dennis and Tara-Kay Booth PO Box 4338 Gypsum, CO 81637 Via email: tarakaybooth14@gmail.com ddennison@hceng.com Subject: Final Acceptance Letter Booth Residence 147 Mustang Mesa Trail Rifle, CO 81650 OWTS Permit #: SEPT-7619 March 31, 2025 Dennis and Tara-Kay, On October 31, 2023, we visited the Booth Residence at 147 Mustang Mesa Trail near Rifle in Garfield County, Colorado to observe the OWTS installation pursuant to OWTS Permit # SEPT- 7619. At the conclusion of our site visits, the contractor had installed the new poly 1,000-gallon two-compartment septic tank, the new effluent line, the distribution box, and the three gravelless chamber trenches. The septic tank was installed level with access brought to grade. An effluent filter had been installed in the outlet tee of the septic tank. The trenches appeared to be level and consisted of 13 ARC34 chambers per trench (39 total chambers, 585 ft2 total additional STA). Additionally, inspection ports were present at each end of each trench. We received a certification letter and as-built drawing regarding the construction of the OWTS from the designer dated March 21, 2025, that indicated the OWTS was installed in general conformance with the permitted design and is functioning as intended. In our opinion, this system was installed in general conformance with Garfield County OWTS Regulations. No further action or review from Garfield County Environmental Health is required regarding this permit. Thank you, Edward R. “Ted” White, P.E.
