CBO lnc. performed a subsurface investigation and compieted an onsite wastewater treatment system
(OWTS) design for the subject r*idence. The 4.63acre property is located outside of Carbondale, in an
area where OWTSs and wells are necessary.

A sample of the soil was taken from Test Pit #2 at S'fe€fr belorru grade^ Soit strucfure grade was
moderate. The ssil formed a ball and a ribbon less than ljnch in length before breaking. Soil structure

The 4-inch SDR-35 sewer line exiting the ADU must have a double-sweep clean out and a minimum 2%
grade to the septic tank.

The control panel for the pump must be located within line of sight of the septic tank. A l0O&gallon, two-

Effluent will be pumped through a 1-S-inch diameterpump line from the pump chamber to an Orenco@
automatic distributing valve (ADV), model 6403" This pump line must have a minimum t% grade for proper
drain back into the tank after each pump cycle- The ADV must be placed at a high point in the system in an
insulated riser with access trom grade. Screened rock must be placed below the ADV to support the ADV
and tO assure the clear pipes exifng the ADV rernain visible for future inspection and maintenance.
Effluent will be pressure dosed through 1.S-inch diameter pipes to three gravelless chamber trenches,
each 8 'Quick 4' PIus Standard lnfiltrator@ chambers, for a total of 24 chambers and 288 sguare feet oJ
infiltrative area- There must be at least*{eet of undisturbed soil behveen each trench..
Effluent will be pressuro dosed to 1.5-inch diameter laterals, which must be hung with zip ties from the
underside of the chambers. Laterals will have 5/32-inch diameter orifices facing up, with the exception of
the first and last orifices tacing doram for drainage" Orenc@ Orifice $hields may be placed under each
downward-facing orifice- The oriftces must be placed Z-teelton center- Each lateral must end in a 90
degree ell facing up with a ball valve for ffushing- Valve may be placed in a vafue box, accessible from
grade, for access- lnspection ports must be placed atthe @inning and end of each trench. Ports may
be cut to grade and covered with a valve box for acc€ss.

INSTALLATION OBSERVATIONS
CBO lnc. must view the OWTS during construction. The OWTS observation should be performed before
backfill, after placement of OTVTS cornponer*s. Septic tanhs, distribution devices, pumps, dosing
siphons, and other gumbing, as applicable, must also be obserued. CBO Inc- shouH be notified 48 hours
in advance to observe the installation,
ln an effort to improve the accuracy of the record drawing, we request that the installer provide a
sketch of the installation, including path of the server lines, water line installation {if applicable),
septic tank location, STA location, and measurements from building cornens or another fixed
objects on the property, Tbis sketch is rnoet eaily provirled on $heet \lE-O of the OWTS Design
Packet. Photographs of the installation and final sover are alsc requested to supplement our installation
documentation-
REI/EGETATION REQUIREMENTS
An adequate layer of good quality topsoil capabb of supporting revegetation shall be placed over the entire
disturbed area of the OWTS lns*allation- A mbdure of natiue grass sd thd has good soil stabilizing
characteristics (brrt without bprwts), provides a maximum transpiration rate" and competes well with
successionat species. No bees or shrubs, or arry vq#tion requirtryl regular in$ation shall be placed over
the STA. Until vegedation is reestablished, erosion and sediment confrol mffisures shall be implemented
and maintained on site. The owner of the OWTS shall be responsible for maintaining proper vegetation
cover.
OPERATTON INFORHATIOH AHD MAINTET{ATGE
The property o$,ner shatl be responsibb for the operation ard rnaintenanoe of emh OWT$ servicing the
property. The property owner is responsible for maintaining service contracts for manufactured units,
alternating STAs, and any other comporents reeding maintenance.

should be plaed on the baokftlled STA- Machines with tracks (not wheets) should be used during

running toild or leaky faucet can discfiarge hundrcds of gallons of water a day and harm a STA.

The homeowner should pump the septic tank every two years, or as needed gaugd by measurement of
solids in the tank. Garbage disposal use should b minimized, and non-biodegradable matedals should not
be placed into the OWTS. Grease should not be placed in housetroH drains- Loding fom a water softener
should not be dlscharged into the OWTS, No haardous wasts shouH be directed into the OWTS.
Mechanical room drains should not discfiarge intotlre OWTS- Tfte OWTS is engineerd for domestic waste
only.
ADDITIONAL COH$TRUCTIOH NOTES
lf design includes a pump, weep holes rnust be installed to allow purnp lines to drain to minimize risk of
freezing. The pump shall have an audible arxJ visual alarm notification in the event of erccessively high-
water conditions and shall be connected to a control breaker separate Fom the higtrwater alarm breaker
and from any other control qrstem circuits- The pump system shall have a switch $o the pump can be
manually operated.

Excavation equipment must not drive in the excavation of the STA due to the potential to compact soil.
Extensions should be ptaced on all septic tank components to allow access to them from existing grade.
Backfill over the STA must be uniform and granular with no material greater than minus 3-inch.
LIMITS:
The design is based on information submitted- lf soit conditions encountered are different fiom conditions
described in report, CBO lnc. should be notified. All OWTS construction must be according to the county
regulations. Requirements not specified in this report rnust follow applicable county regulations. The
contractor should have rlocrrmented and demonstrated knowledge of the requirements and regulations of
the county in which they are working. Liensing of Systems Contractors may be required by county
regulation.
