Box 342 Avon, CO 81620 Via email: tedvic@comcast.net , kaglenwood@kumarusa.com Subject: Final Acceptance Letter TBD County Road 150 Gypsum, CO 81637 OWTS Permit #: SEPT-9072 August 12, 2025 On May 8, 2025, we visited the site at TBD County Road 150, Parcel Number # 186323200099 near Gypsum in Garfield County, Colorado to observe the OWTS installation pursuant to OWTS Permit # SEPT-9072 (concurrent with Building Permit BLRE-9071). At the time of our site visit, the contractor had installed the new sewer line and cleanout, the new poly Infiltrator 1,060- gallon two-compartment septic tank with an effluent filter in the outlet tee of the tank, the new effluent line, the new distribution box, and two 60 ft long gravelless chamber trenches. The septic tank and distribution box were installed level with access brought to grade. The septic tank utilized for this project is on the accepted septic tank list published by the Colorado Department of Public Health and Environment. Each of the two trenches consisted of 15 ‘Quick 4’ chambers (30 total chambers, 360 ft2 total STA). Additionally, inspection ports were present at each end of each trench. At the time of the inspection a rise was observed in the southern trench; the distribution box was observed to be out of level, and watertight risers, inspection port caps, and an effluent filter handle extension had yet to be installed. Garfield County Public Health received photo documentation that these items had been resolved per Garfield County OWTS Regulation 43 on May 16, 2025. We received an as-built drawing of the constructed system and a letter certifying that the system was constructed in compliance with the permitted design from the OWTS designer dated August 7, 2025. In our opinion, this system was installed in general conformance with Garfield County OWTS Regulations. No further action or review from Garfield County Environmental Health is required regarding this permit. Thank you, Julia C.
