Permit Conditions:
This OWTS Permit is approved on the condition of compliance with the report and design as submitted by
Huddleston-Berry Engineering dated September 3, 2025. Changes to the design and permit must be approved
by Garfield County Public Health prior to construction.
The new OWTS will expand an existing STA and serve a new four-bedroom residence. The existing sewer line
to the existing residence must be removed. The existing water line crossing the proposed sewer line must
be removed. The sewer line for the new residence will tie into the existing sewer line upstream of the
existing septic tank. The sewer line must be incased along all sections that encroach on the 25ft setback
from the water supply line. The existing 1250-gallon two compartment septic tank set level with access to
grade and an effluent filter installed in the outlet tee of the tank will remain in use. Effluent will gravity flow
to the first existing trench in the serial distribution system that comprises the STA. The STA will contain a total
of 744 ft2 of infiltrative surface comprised of 480 ft2 of existing STA and 264 ft2 of newly installed STA. The
existing STA area is previously undocumented and is comprised of four 40 foot long gravelless chamber
trenches in serial configuration. All existing sewer and distribution pipes must be in good working order and
any damaged pipe or components must be replaced. The new STA area will consist of three additional
gravelless chamber trenches in serial configuration installed downstream of the existing trenches: two
trenches of 10, and one trench of 2, ‘Quick 4’ chambers (22 chambers total). Inspection ports will be installed
at each end of each trench. Maintain all setbacks and elevations as designed.
Contact the designer and Garfield County Public Health at least 48 hours prior to requesting final inspection
of the installed system prior to backfill. NO OWTS COMPONENT SHALL BE BACKFILLED WITHOUT PRIOR
APPROVAL FROM GARFIELD COUNTY PUBLIC HEALTH AND THE DESIGNER.
Agreement
All installations must comply with all requirements of the Garfield County OWTS regulations
including but not limited to the conditions specified above.
In consideration of the issuance of the permit, the applicant hereby agrees to comply with all
laws and regulations related to the zoning, location, construction, and erection of the proposed
OWTS and associated structures for which this permit is granted. The applicant further agrees
that if the above said regulations are not fully complied with in the zoning, location, erection,
and construction of the above described OWTS and structure, the permit may then be revoked
by notice from the Health Department and immediately become null and void.
The issuance of a permit and specifications of terms and conditions therein will not constitute
assumption of liability, nor create presumption that the local public health agency or its
employees may be liable for the failure or malfunctioning of any system. Permit issuance will
not constitute a certification that the system, the equipment used in the system, or any
component used for system operation will ensure continuous compliance with the provision of
the OWTS Act, the regulations adopted thereunder, or any terms and conditions of a permit.
Permit Approval
Plans and specifications for the proposed OWTS have been reviewed and are considered
satisfactory. Permission is hereby given to the property owner(s) to perform the work indicated
in accordance with the Garfield County OWTS Regulation. This Permit will expire 1 year from
the date of issuance unless construction on the system has commenced. An "as-built" drawing
must be submitted and inspection of the system must be completed by GCPH before final
approval of the system will be issued.
