Our observations and recommendations are presented in this report. The services are supplemental to Kumar & Associates agreement for professional services with R.C. Schneider Construction dated December 16, 2024. We previously designed the OWTS for the residence as submitted in our report dated March 19, 2025, Project No. 24-7-702.02. The system was designed for three bedrooms. We performed our as-built evaluation on August 12, 2025. The OWTS components observed included: cleanout, sewer line, septic tank, pump, effluent line, and soil treatment area (STA) and the locations of these components. The installation and locations of the system components were verified as shown on Figure 1. Pictures of the OWTS components made during the site visits are included with this report. The distance locations to the septic tank shown on Figure 1 were made by taping. The septic tank was changed from the proposed Valley Precast 1,000-gallon, two-chamber tank to a Norwesco/Snyder 1,500-gallon, two-chamber septic tank. The proposed Tuf-Tite 4-hole distribution box was changed to a Tuf-Tite 7-hole distribution box. The proposed 2 rows of 22 Infiltrator Quick4 Plus Standard chambers (44 chambers, 528 square feet) were changed to 2 rows of 18 Arc36 chambers (36 chambers, 540 square feet). The changes are acceptable to us. It is our opinion that the OWTS was installed in general conformance with the above-referenced design. The observations and recommendations presented above are based on our site observations and our experience in the area. We make no warranty either express or implied. Alterations to the system or improper backfill and site grading could affect the operation and effectiveness of the OWTS.
