Installation was authorized under Garfield County Public Health Permit # SEPT-9278. Approved design plans were used for construction of the replacement OWTS and construction inspections were completed by Eric Blake, P.E. The OWTS was installed in general conformance with the design plans, with approved substitutions as follows: • Arc 36 chambers were installed instead of the specified Quick4 chambers. This resulted in a total of 160 chambers installed in 8 rows instead of the specified 200 chambers installed in 8 rows. The Arc 36 chambers have a longer chamber length, and therefore, the specified total row length of 100 feet was still achieved as well as the necessary 2,400 square feet of soil treatment area. • A Valley Precast 2000T-2CP was installed instead of the Valley Precast 2000T-1CP. Both tanks are on the approved list from CDPHE’s list of Accepted Septic Tanks for Use in Colorado On-Site Wastewater Treatment Systems and meet the requirements of 43.9 B of Garfield County On-Site Wastewater Treatment Systems (OWTS) Regulations. • The effluent filter identified in the design plans was determined to be undersized prior to construction. An Orenco FT0854-36A effluent filter was installed in order to accommodate the 1,970 gpd design flow. All installation was functioning as intended and installed in conformance with the plans and design intent. As- Built documents identifying the above-mentioned changes are attached. Sincerely, Eric Blake, P.E. Northwest Region Project Manager
