Garfield County Public Health (GCPH) staff visited the site of the residence at 310 Flying Fish Road, Carbondale,
Colorado on multiple occasions to observe the OWTS installation pursuant to OWTS Permit #SEPT-9195
(concurrent with BLRE-9040).
August 1, 2025: Initial inspection noted installation of main residence sewer lines, 1530-gallon septic tank,
552-gallon pump chamber, high-head pump, pump line, ADV, manifolds, and 940 ft2 STA consisting of
two, 3ft deep, sand filter beds with GeoMat (lower infiltrative surface at 2ft below grade). Deficiencies
included unwired pump; missing ADU sewer line, inlet tee, flushing valves, effluent filter, and inspection
ports; unanchored tanks; improper pipe and manifold slopes; continued use of existing system, and lack of
ET bed liner removal confirmation. Approval of backfill was denied.
August 6, 2025: Reinspection confirmed proper installation of sewer line, manifolds, inlet tee, effluent
filter, inspection ports, and flushing valves. The existing STA had been disconnected and abandoned in
place. Remaining deficiencies included unwired pump, tanks anchored and backfilled without inspection,
improper ADV and pump line slopes, and a dewatering well installed within setbacks.
September 4, 2025: Final inspection and email correspondence verified relocation of the dewatering well,
corrected ADV and pipe slopes, proper pump wiring, functional alarms, dead-men weight, proper
abandonment of existing septic tank, and appropriate mounded cover.
We received an installation observation letter and As-Built drawing from the system designer (dated September 10,
2025) indicating the OWTS was installed in general conformance with the permitted design, that all tanks had been
anchored per manufacturer specifications, and that no ET bed liner was recovered despite efforts made to locate it.
In our opinion, this system was installed in general conformance with Garfield County OWTS Regulations.
No further action or review from Garfield County Environmental Health is required regarding this permit.
