On August 6th, 2025, Garfield County Public Health (GCPH) staff
performed a second final system installation inspection of a new
Onsite Wastewater Treatment System (OWTS) on the property
located at 310 Flying Fish Road near Carbondale in Garfield
County, Colorado associated with OWTS Permit No. SEPT-9196.
A final letter was issued on November 18, 2025. This letter
provides notice that you have met all the requirements of the Notice
of Violation issued by GCPH on May 5th and revised August 1,
2025, and that the NOV is hereby RESOLVED.
