THE TCB REVOCABLE TRUST
Pursuant to CRS §11-105-111
The undersigned hereby attests and certifies as follows:
1. The name of the trust to which this affidavit relates (the "Trust") is the TCB Revocable Trust.
2. The Trust exists. The trust agreement was executed on aq ( a-4.
3. The settlor of the Trust is Lisa Suzanne Warnick.
4. The identity and address of the sole, currently acting trustee of the Trust (the "Trustee") is as
follows:

5. The Trust is revocable and Lisa Suzanne Warnick has the power to revoke it.
6. The Trustee has all powers given to him by the laws of Colorado and as further
enumerated by the trust instrument, including without limitation (a) the power to access,
control and transfer bank accounts, (b) the power to open bank accounts, and (c) the
power to hold, retain, and manage trust assets; and (d) the power to transfer real property
to the trust.
7. The taxpayer identification number of the Trust is 219-08-5760

9. The Trust has not been revoked, modified, or amended in any manner that would cause the
representations contained in this certification of trust to be incorrect.
10. I am the only currently acting trustee.

all of the statements in this certification are true, complete and accurate.
