CBO Inc. observed the installation of the onsite wastewater treatment system (OWTS) on December 23,
2025 for the subject property. Silt Mesa Excavation installed the system.
The property is currently developed with a 4-bedroom residence. The residence is served by an existing
OWTS documented by Garfield County OWTS Permit No. SEPT-6166, issued March 24, 2020.
The existing soil treatment area (STA) was sized for use from the proposed Barn. The proposed location
and elevation of the Barn required a new septic tank and pumping system to utilize the existing STA.
The new 4-inch SDR-35 sewer line exiting the residence Barn had a double-sweep clean out and a
minimum 2% grade to the septic tank.
The system installation included a 1000-gallon, two-compartment Infiltrator® poly septic tank with an
Orenco® Biotube effluent filter on the outlet tee. This filter must be cleaned annually, or as needed.
Effluent gravity flows to a 300-gallon 1, single-compartment Infiltrator® pump chamber with a F&M ½ HP
sewage ejector pump, Model 40ECF. The floats were set to dose approximately 60 gallons each pump
cycle, allowing approximately 16.5 gallons of drain back. The control panel for the pump was located
within line of sight of the septic tank. The pump and alarm were tested via the floats and operated properly.
Effluent is pumped through a 1.5-inch diameter pump line from the pump chamber to the existing sewer
line entering the existing the distribution box. This pump line must had a minimum 1% grade for proper drain
back into the pump chamber after each pump cycle.
The OWTS was generally installed according to specifications. This observation is not a guarantee of
workmanship and/or parts and materials. CBO Inc. should be notified if changes are made to the OWTS
in the future. Any additional OWTS construction must be according to the county regulations.

LIMITS: Observations are limited to components that are visible at the time of the inspection. The installer must
have documented and demonstrated knowledge of the requirements and regulations of the county in
which they are working. The quality of the installation is dependent on the expertise of the installer, soil
type, and weather conditions.

Permit Conditions:
This OWTS Permit is approved on the condition of compliance with scope of work and site plan as
submitted by CBO, Inc. on November 26, 2025. Changes to the design and permit must be approved
by Garfield County Public Health prior to construction.
This permit allows a new connection to the existing OWTS at the property. A proposed one-bedroom
ADU necessitates a new connection to the system. The new connection from the ADU will include a
new sewer line with a cleanout out located within 5ft of the outside of the proposed building. The
new sewer line will flow to a new 1000-gallon two compartment septic tank on the CDPHE approved
list equipped with watertight risers and an effluent filter in the outlet tee followed by a 300-gallon
single compartment pump chamber with a F&M ½-HP sewage ejector pump in the chamber. The
pump floats will be set to dose approximately 60 gallons each pump cycle, allowing approximately
16.5 gallons of drain back. Effluent will be pumped through a new 1.5-inch diameter pump line to a
new connection with the existing effluent line before the existing distribution box where it will be
distributed to the existing STA. Maintain all setbacks and elevations as designed.
To assist in scheduling, please contact Garfield County Public Health (970-945-6614 ext. 8150) to
notify the start of OWTS construction. Contact the designer and Garfield County Public Health at
least 48 hours prior to requesting final inspection of the installed system prior to backfill. NO OWTS
COMPONENT SHALL BE BACKFILLED WITHOUT PRIOR APPROVAL FROM GARFIELD COUNTY PUBLIC
HEALTH AND THE DESIGNER.
Agreement
All installations must comply with all requirements of the Garfield County OWTS regulations
including but not limited to the conditions specified above.
In consideration of the issuance of the permit, the applicant hereby agrees to comply with all
laws and regulations related to the zoning, location, construction, and erection of the proposed
OWTS and associated structures for which this permit is granted. The applicant further agrees
that if the above said regulations are not fully complied with in the zoning, location, erection,
and construction of the above described OWTS and structure, the permit may then be revoked
by notice from the Health Department and immediately become null and void.
The issuance of a permit and specifications of terms and conditions therein will not constitute
assumption of liability, nor create presumption that the local public health agency or its
employees may be liable for the failure or malfunctioning of any system. Permit issuance will
not constitute a certification that the system, the equipment used in the system, or any
component used for system operation will ensure continuous compliance with the provision of
the OWTS Act, the regulations adopted thereunder, or any terms and conditions of a permit.
Permit Approval
Plans and specifications for the proposed OWTS have been reviewed and are considered
satisfactory. Permission is hereby given to the property owner(s) to perform the work indicated
in accordance with the Garfield County OWTS Regulation. This Permit will expire 1 year from
the date of issuance unless construction on the system has commenced. An "as-built" drawing
must be submitted and inspection of the system must be completed by GCPH before final
approval of the system will be issued.
