Elk Mountain Excavation installed the system. The OWTS design is based on 4-bedrooms. An average daily wastewater flow of 525 GPD was used. Camera work was completed to locate the existing STA and no drywell or seepage pit was found. Camera work indicated a possible pipe and gravel seepage bed, which was abandoned in place. The existing sewer line from the house to the septic tank and existing 1500-gallon, two-compartment Copeland Concrete septic tank remained. A new sewer line was installed to a new 500-gallon, single- compartment Infiltrator® poly pump chamber with an Orenco® Biotube Pump Vault and an Orenco® PF3005 pump. The floats were set to dose 71 gallons each pump cycle. The control panel for the pump is located within line of sight of the septic tank. Valley Precast out of Buena Vista performed start-up of the pumping system. This office was present for the squirt test and cycled through the automatic distributing valve (ADV). Effluent is pumped through a 1.5-inch diameter pump line from the pump chamber to an Orenco® ADV, model 6402. The ADV is placed at a high point in the system in an insulated riser with access from grade. Effluent is pressure dosed through 1.5-inch diameter pipes to two gravelless chamber beds, each with 2 rows of 13 ‘Quick 4’ Plus Standard Low Profile Infiltrator® chambers, for a total of 52 chambers and 624 square feet of infiltrative area. Effluent is pressure dosed to 1.5-inch diameter laterals, which were hung with zip ties from the underside of the chambers. Laterals have 5/32-inch diameter orifices facing up, with the exception of the first and last holes facing down for drainage. Orenco® Orifice Shields are placed under each downward-facing orifice. The orifices were placed 2.5-feet on center. Each lateral ends in a 90 degree ell facing up with a ball valve for flushing. Valves are placed in a valve box, accessible from grade, for access. Inspection ports were placed at the beginning and end of each trench. Ports are cut to grade and covered with a valve box for access. The depth of the infiltrative area did not exceed 2.5-feet below native grade. LIMITS: Observations are limited to components that are visible at the time of the inspection. The installer must have documented and demonstrated knowledge of the requirements and regulations of the county in which they are working. Changes to the design and permit must be approved by Garfield County Public Health and the designer prior to construction. The new OWTS will serve a four-bedroom residence. The system will consist of an existing two-compartment 1,500-gallon septic tank with access at grade that has been found to be in good condition. All sewer lines must be found in good working order or replaced. A new double sweep cleanout will be installed in the existing sewer line prior to the existing septic tank. A new screened high-head pump will be housed in a 500-gallon single-compartment pump chamber downstream from the septic tank. The dose volume from the pump shall be 71 gallons pumped ~7 times per day. The specified pump for the project must have a minimum 28.6 gallon-per-minute pump rate and a total dynamic head of 23 feet. Effluent will be pumped via a 1.5 inch pressure transport line to an ADV at the high point of the system that will distribute effluent to 624 ft2 of STA. The infiltrative surface must be installed no deeper than 2.5 feet below native grade to provide adequate separation from R-0 soils beginning at 6.5 ft below native grade. The new STA will consist of two 6 ft by 52 ft chamber beds. Each bed will consist of 2 rows of 13 ‘ARC 36’ chambers (52 chambers total). Distribution laterals are to be 1 1/2 inch Schedule 40 PVC, with 5/32-inch orifices every 2.5 -feet on center and the laterals ending in 90-degree sweeping bends and flushing valves at the distal end. The orifices are to be facing upward, except for the last orifices (which shall face downward and be equipped with orifice shields). Inspection ports are to be installed in each corner of each bed. The residual head (squirt height) at the end of the system is to be 5-feet. Maintain all setbacks and elevations as designed. To assist in scheduling, please contact Garfield County Public Health (970-945-6614 ext. 8150) to notify the start of OWTS construction. Contact the designer and Garfield County Public Health at least 48 hours prior to requesting final inspection of the installed system prior to backfill. NO OWTS COMPONENT SHALL BE BACKFILLED WITHOUT PRIOR APPROVAL FROM GARFIELD COUNTY PUBLIC HEALTH AND THE DESIGNER. Agreement All installations must comply with all requirements of the Garfield County OWTS regulations including but not limited to the conditions specified above. In consideration of the issuance of the permit, the applicant hereby agrees to comply with all laws and regulations related to the zoning, location, construction, and erection of the proposed OWTS and associated structures for which this permit is granted. The applicant further agrees that if the above said regulations are not fully complied with in the zoning, location, erection, and construction of the above described OWTS and structure, the permit may then be revoked by notice from the Health Department and immediately become null and void. The issuance of a permit and specifications of terms and conditions therein will not constitute assumption of liability, nor create presumption that the local public health agency or its employees may be liable for the failure or malfunctioning of any system. Permit issuance will not constitute a certification that the system, the equipment used in the system, or any component used for system operation will ensure continuous compliance with the provision of the OWTS Act, the regulations adopted thereunder, or any terms and conditions of a permit. Permit Approval Plans and specifications for the proposed OWTS have been reviewed and are considered satisfactory. Permission is hereby given to the property owner(s) to perform the work indicated in accordance with the Garfield County OWTS Regulation. This Permit will expire 1 year from the date of issuance unless construction on the system has commenced.
