All installation must comply with all requirements of the County Individual Sewage Disposal Regulations, adopted pursuant to au- thority granted in 66 -44 -4, CRS 1963, amended 66 -3 -14, CRS 1963. 2. This permit is valid only for connection to structures which have fully complied with County Zoning and building requirements. Connection to or use with any dwelling or structures not approved by the building and Zoning office shall automatically be a viola- tion of a requirement of the permit and cause for both legal action and revocation of the permit. 3. Section III, 3.24 requires any person who constructs, alters, or installs an individual sewage disposal system in a manner which in- volves a knowing and material variation from the terms or specifications contained in the application of permit commits a Class I, Petty Offense ($500.00 fine — 6 months in jail or both. COLORADO DEPARTMENT OF HEALTH DIVISION OF ENGINEERING AND SANITATION ACTIVITY REPORT Code Section Individual Sewage County Garfield FILE REFERENCE: Individual Sewage Disposal Systems INDIVIDUAL OR ESTABLISHMENT: Warren Walker ADDRESS: 0703 County Road 250 - New Castle, Colorado 81647 NARRATIVE: Introduction: Mr. Bud Milner, the Garfield County Building Official, informed this representative that on an inspection call on Silt Mesa he had observed what he thought to be a possible septic system being installed at the above referenced address. Mr. Milner inquired if we were aware of this particular construction effort and we informed him that we did not have a permit on the system if it were in fact a septic system. An inspection of the property was scheduled to determine if there was a septic system being installed without a permit at the said address. Findings: March 25, 1976, 9:00 A.M. - This representative arrived at the property and made the following observations: 1. Two trenches had been dug approximately 35 feet long and 2 feet wide. 2. A small pile of 11 inch to 2 inch aggregate was observed beside the trench excavation. 3. A pipe (4" PVC) had been connected to the outlet line of a Copeland Concrete Septic Tank. This pipe led to the beginning of the trenches. 4. Septic tank effluent of obvious color and odor was observed coming from the newly extended pipe and was draining into the newly excavated easterly trench. Action Taken: This representative found Mrs. Walker to be home and informed her of the violation that existed. She stated she nor her husband had realized that a permit was necessary to repair a septic system. She offered their willing compliance and promised to fill out and submit the application packet which this officer left with her at the property. Conclusion: Mr. & Mrs. Walker submitted their application for a repair permit and the applicable fee of $50.00 for the percolation test to this officer. Construction of a conforming nature is presently under way and a permit has been issued for the repair of the system. LETTER TO FOLLOW: ( ) OTHER RECOMMENDATIONS: DATE: March , 19 .
