All installation must comply with all requirements of the County Individual Sewage Disposal Regulations, adopted pursuant to au- thority granted in 66-44-4, CRS 1963, amended 66 -3 -14, CRS 1963. 2. This permit is valid only for connection to structures which have fully complied with County Zoning and building requirements. Connection to or use with any dwelling or structures not approved by the building and Zoning office shall automatically be a viola- tion of a requirement of the permit and cause for both legal action and revocation of the permit. 3. COLORADO DEPARTMENT OF HEALTH DIVISION OF ENGINEERING AND SANITATION ACTIVITY REPORT Code Section Milk, Food & Drug County Garfield FILE REFERENCE: Individual Sewage Disposal Systems INDIVIDUAL OR ESTABLISHMENT: William R. Keating ADDRESS: 0483 167 Road - Glenwood Springs, Colorado 81601 NARRATIVE: Introduction: April 21, 1976 - 10:30 A.M. - This department was notified by telephone on two separate occasions of a construction project believed to be involving a sewage disposal system that was being improperly installed at the above address. Mr. Bud Milner, the Garfield County Building Inspector, and the undersigned proceeded to the address to investigate the situation regarding not only the possibility of illegal sewage system installation but also for the possibility of no building permit. Findings: The construction project was underway on the adjoining property to the above addressed property which was being conducted by the owner Mr. Ernie Proffit of Box 3907 - Aspen, Colorado. Upon talking with Mr. Proffit, he assured us he was only bulldozing his driveway and he had every intention of getting the proper permits prior to construction of his new home. Proper applications were left with Mr. Proffit and Milner and Feld left that particular property. Feld later inspected the adjoining property to Proffit's at the above listed address and discovered the source of the sewage complaint. Additional Findings: Mr. Keating had proceeded with a repair to his existing septic tank system without the appropriate permits and proper design. Mr. Keating had made an excavation approximately 10' to 12' deep and 10' to 12' square and had deposited numerous steel and metal debris in addition to a discarded and junked car body into the hole. He intended to use a make -shift seepage pit (drywell). After observing the improper and unacceptable construction methods being undertaken at the site, this representative talked with Mrs. Keating regarding the situation. An Official Notice was left informing them of the violation and also an application for a repair permit was left with Mrs. Keating. April 26, 1976, Mrs. Keating brought the application into the Garfield County Environ- mental Health Dept. and made appropriate application for a legal repair of their septic tank system. REPRESENTATIVE: ��/ i/ t 4, Acting D=':rtment Head ES: 7 (Rev. 6 -70 -100) Page Two Action Taken: At the time of the observation of the illegal system being installed it was requested by this officer and complied with by the Keatings that no more work be done on that particular system until the appropriate permits could be issued and a redesign of the system be setforth. Conclusion: Mr. Keating was issued Permit No. 268 for a repair to his individual sewage disposal system for a 24' x 39' x 3' seepage bed in an acceptable area of the property. Construction is presently underway and the Keatings are cooperating fully with this office. Individuals who originally voiced the complaint were notified of the actions taken by this office.
