At the location near the convenience store the profile hole excavation encountered a sand/gravel at ±5' depth to the bottom (10' -11') of the excavation. This material had a percolation rate of 3 -4 minutes per inch. The soil above the sand /gravel has an average percolation rate of approximately 35 minutes per inch. At the easternmost location, the sand/gravel was encountered at 1 1'-12' depth in the profile hole. Percolation rates were averaged in three (3) percolation holes at 3' -4' depth -with the rate being approximately 16 minutes per inch. These percolation holes were then deepened to ±6' and the average percolation rate at this depth was approximately 52 minutes per inch. If you need any further information, let me know. In the course of this review the Division has Identified a number of Issues which must be resolved before favorable consideration can be given to your proposal. These issues are described below. You have failed to sign the application and certification at the bottom of page three. The application form is therefore being returned to you. The application Is for two phases of construction. The timing of the second phase Is at some unknown point In the future as demand warrants'. The Division may consider applications for facilities which will be under construction within one year of approval. Unless you are committed to constructing both phases of the wastewater system at this point in time, It Is suggested that Phase 11 be withdrawn from this application. A separate site application will be required for each construction phase. The facility is identified as being non - discharging. This is no longer the case. Effective July 1, 1993, facilities discharging to groundwater will be required to obtain discharge permits if their design capacity exceeds 2,000 gallons per day. The application contains no evidence of control of the site. The Division must have a copy of appropriate documentation (le. deed, easement, lease, etc.) demonstrating legal authority to use the site for this purpose. The description of the selected alternative does not identify the number and location of percolation tests performed. It also falls to address the required eight foot soil profile and the depth to seasonal high groundwater level. The financial system fails to address or demonstrate the existence of adequate financial capabilities for the construction, operation, and maintenance of the system. The Implementation plan fails to address the estimated start-up date for the system. The distance separating the Phase I absorption bed from the Colorado River (258 feet) and the Phase 11 absorption bed from the well (220 feet) as shown on the I.S.D.S. Site Plan dated 1/11/93 do not meet minimum criteria established by the Colorado Board of Health. Setback distances are based upon the design capacity (maximum flow) of the system. Phase 1I, with an average daily flow of 2,500 gallons and a maximum flow (design capacity) of 3,750 gpd, requires a separation of 320 feet from any well.
