1. All Installation must comply with all requirements of the Colorado State Board of Health Individual Sewage Disposal Systems Chapter
25, Article 10 C.R.S. 1973, Revised 1984.
2. This permit is valid only for connection to structures which have fully complied with County zoning and building requirements. Con-
nection to or use with any dwelling or structures not approved by the Building and Zoning office shall automatically be a violation or a
requirement of the permit and cause for both legal action and revocation of the permit.
3. Any person who constructs, alters, or installs an individual sewage disposal system in a manner which Involves a knowing and material c
variation from the terms or specifications contained in the application of permit commits a Class I, Petty Offense ($500.00 fine — 6

Individual Sewage Disposal Systems
Dear Scott:
The purpose of this letter is to summarize the results of percolation test-
, ing done previously by Lincoln- De'ore and the feasibility of installing
individual sewage disposal systems (ISU) on the above referenced property.
The Lincoln -DeVore report indicated that percolation rates varied between 3
min /inch and 60 min /inch over the property. A total of nine holes were
constructed and tested. Based upon this information, as well as a visual
examination of the property, it appears the percolation tests would be
representative of anticipated percolation rates in the observed soil types.
'There is no reason not to assume that similar percolation rates will be
found on each of the proposed 18 lots.
It is anticipated that standard septic tank /leach field systems can be
constructed for a majority of these lots. In the event site specific
restraints are found during the construction on a particular lot, it may be
necessary to construct an engineered system, either a Wisconsin Round sys-
tem or an evapo- transpiration (E1') system. Restraints which may occur would
be excessive percolation rates, inadequate percolation rates or bedrock
conditions. None of the sites would preclude an engineered system from
being constructed.
Attached hereto is a proposed management plan for ISC6's. If followed, the
management plan would assure that the septic tanks were pumped out on a
• regular basis, but the purpose of the management plan is not to guarantee
a particular system against failure or to provide a funding mechanism for
repair of failed systems.
I will remain available to provide additional information as needed. 'Ibis
letter will serve as an exhibit for the Preliminary Plat Submission bo

MANAGEMENT PLAN FOR INDIVIDUAL SEWAGE DISPOSAL SYSTEMS
A. Purpose
The purpose of this management plan is to provide for regular oper-
ation and maintenance of the individual sewage disposal systems.
The management plan provides a mechanism for regular pumping of
septic tanks and for funding the cost thereof. This management
plan is not intended to provide for common ownership of sewage dis-
posal facilities, nor to provide a mechanism for funding for, or
the actual construction of, replacement of individual systems.
B. Responsibility of Management Plan
The management plan shall be the responsibility of the Pinyon Peaks
Homeowners Association. The Homeowners Association shall make
arrangements with a septic tank pumping company for the pumping of
septic tanks on a two -year, rotating basis.
C. Funding
The Pinyon Peaks Ebmeowners Association will collect, as part of
the Association dues, sufficient funds to pay for the bi- annual
pumping of septic tanks. The amount of funds collected shall be
adjusted as necessary to pay for the cost of the management plan.
D. Individual Homeowner Responsibilities
1. Provide access to the septic tank for purposes of cleaning.
2. Pump septic tank more frequently, if required, based on actual
use.
3. Initially install, and subsequently replace, failed leach field
systems as required, all in accordance with applicable Garfield
County Health Codes.
