1. All installation must comply with all requirements of the Colorado State Board of Health Individual Sewage Disposal Systems Chapter
25, Article 10 C.R.S. 1973, Revised 1984.
2. This permit is valid only for connection to structures which have fully complied with County zoning and building requirements. Con-
nection to or use with any dwelling or structures not approved by the Building and Zoning off ice shall automatically be a violation or a
requirement of the permit and cause for both legal action and revocation of the permit.
3. Any person who constructs, alters, or installs an individual sewage disposal system in a manner which involves a knowing and material
variation from the terms or specif ications contained in the application of permit commits a Class I, Petty Offense ($500.00 fine —8
months in )ail or both).

(1) Building Permit - According to our records, we have not performed a requested
inspection on the bunk house since September 9, 1995, a drywall inspection. The building codes
adopted by Garfield County specify that a building permit expires after 180 days if work is
suspended or abandoned. Also, a final inspection by Garfield County is required before any use
or occupancy is allowed. A final electrical inspection by the state inspector is also required before
an:. use and occupancy. Any use or occupancy before recJving the required final inspections is
considered to be illegal. If the bunkhouse is ready for a final, please schedule it as soon as
possible.
(2) Septic System - According to our records, you are to provide us with information
verifying what was installed in the septic system. Apparently we have never received the required
information. Also, the required $50 fee has never been paid. Payment for this permit and
submitting the requested information is required before use and occupancy of the structure is
allowed.
Please give these matters your immediate consideration so that all requirements are
properly fulfilled. Please call the office if you have any further questions. Copies of the relevant
paper work are enclosed.

Dear Ruth & Ray,
(1) Building Permit - Reference previous letter. We are asking for information on the
status of the building ?? If the bunkhouse is ready for a final, please schedule it as soon as
possible. If you have questions - call our office.
(2) Septic System - According to our records, you are to provide us with information
verifying what was installed in the septic system. We received a letter from Mike Frick of Dirt 'n
Iron stating that a 1000 gallon septic tank was installed. Our copy of the septic permit application
states that this system is supposed to be for a four bedroom house. A 1000 gallon septic tank is
only adequate for a 3 bedroom house. Although Mike stated that the existing leachfield appeared
to be working properly and was made of 4" PVC perforated pipe in a gravel bed, no information
was provided on the size and perc rate.
To recap, according to the septic permit, you, the owner are to provide the Garfield
County Building and Planning Department with the following;
(1) Payment of $50 permit fee.
(2) Verification of system location.
(4) Verification of system size and adequacy for 4 bedroom house.
(5) Verification of pert rate used to determine field size.
This information can only be provided to us by a Registered Professional Engineer (P.E.)

discuss what is needed by Garfield County. We would ask that all information for finaling this
structure and septic system verification be provided to the Garfield County Building Department
no later than Friday October 18, 1996. If you fail to provide us with the requested information
by Friday October 18, 1996, we may turn all information over to the County Attorney's office at
that time.

Re: Miller Cattle Company Septic System South Of Silt
Dear Jake,
I installed a new 1,000 gallon septic tank for the Miller's November 1994. The old tank
which was made of brick had failed and was no longer letting anything into the leach field.
The old tank was pumped and a new tank was installed. Also at that time I checked out
the leach field which seemed to be working properly and in sufficient size. The leach field
is made of 4" PVC perforated pipe with a gravel bed and looks to be working fine. The
leach field was installed approximately a year before Miller's had purchased the property.
In my professional opinion the septic system is in perfect working order.
