Step I: Application A. Obtain a standard "Individual Sewage Disposal System" application from the Building and Planning Department, 109 8th Street, Suite 303, Glenwood Springs, Colorado 81601, 970 - 945 -8212 or 285- 7972. B. Return completed application, map to property, site plan and diagram of site to the Building and Planning Department (Pages 2 and 3) and building plans. INDIVIDUAL SEWAGE DISPOSAL SYSTEM PERMITS WILL NOT BE ISSUED WITHOUT AN APPROVED BUILDING PERMIT. C. Make check or money order payable to "Garfield County Treasurer" (The cost for the permit fee and percolation test is $150.00 or if a Registered Professional Engineer does the percolation test, the cost is $50.00 permit fee). FEES ARE NOT REFUNDABLE. D. Your Individual Sewage Disposal Permit will be issued along with your building permit provided no prohibitive problems are encountered. Step II: Percolation Tests -SEE DETAILED INSTRUCTIONS ON PAGE 6 A Prepare three (3) percolation holes 4 feet deep, 8 to 12 inches in diameter, and 20 feet apart in the area of the proposed leach field area (Instructions for post hole or back hoe holes are on page 4). B. Fill percolation holes with water once for the required eight (8) hour soaking period. C. Request percolation test by Sanitarian. (To avoid construction delay, we suggest arrangements for percolation test be made at least 24 hours prior to the end of the soaking period). Please have at least five (5) gallons of water for each hole available at the site for the percolation test. I!!!IMPORTANT! 111 Please be advised that if the Sanitarian's initial field visit to your property reveals any unusual difficulties such as high water table, excessive percolation rates, bedrock, etc., the services of a Colorado Registered Professional Engineer and/or Board of Health approval will be required. Step III: Final Inspection A. When all components are in place, connected and ready to cover, request a final inspection by the Sanitarian. B. DO NOT backfill any part of the system prior to the inspection. C. The initial fee covers the percolation test and one inspection before cover up. Any additional percolation tests will be charged at $100.00 each and additional inspections will be charged at $42.00 each. D. The system was found to conform with the plans and specifications set forth in the engineered sewage disposal system design for the aforementioned property prepared on June 24, 2001. Copies of the engineered plan may be obtained from the undersigned design engineer. Drainage in the area of concern is approximately 8% to the north. A perc test/soils evaluation was conducted on the property by a representative of the Garfield County Building and Sanitation Department on 06/13/01 to provide subsurface design ' parameters for a new individual sewage disposal system (ISDS). Results of the 06/13/01 site investigation revealed a slow perc rate at the site (70 min/in.). In hopes of fording more favorable perc rates at the site, a second perc tests/soils evaluation was conducted at the site by Tom A. Cronk, registered professional engineer 1 (R.P.E.) on 06/22/01. The 06/22/01 perc evaluations were conducted at a deeper depth in the sand soil horizon. Results of the 06/22/01 perc investigation indicate the deeper sand soils are indeed more favorable for subsurface absorption (i.e., faster perc rates). An engineered ISDS design based on results from the 06/22/01 study has been prepared for the property. Both the 06/13/01 and the 06/22/01 site investigations are attached for reference as Appendices A and B respectively. A discussion of developed design parameters from the 06/22/01 investigation and the engineered design follow. 1 2.0 Development of Design Parameters The soils excavation trench associated with the 06/22/01 site study was extended to a depth of 144" below ground surface (BGS). There was no evidence of ground water or high seasonal water table in the open excavation to a depth of 144" BGS. The soils evaluation indicates three distinct soil horizons underlie the site. ' A lithological description follows: depth (in.) description 0" - 16" silty clay, gray; hard and blocky 16" - 60" sandy clay /clayey sand, tan to chalky white; white caliche cementing; hard and smeared 60" - 144" sand, tan to light brown; loose and flowing Four perc holes were constructed at a depth of 76 in. The locations of the excavations are shown on the attached plot plan. An INFILTRATOR absorption field is proposed for the site. As based on the 06/22/01 site investigation (Appendix B), a design perc rate of 25 min/in. is chosen for overall system sizing. 1 1 1 1 1 1 Page 2of8 1
