WHEREAS, Colorado Rocky Mountain School(hereinafter "Owner ")
has applied for a Certificate of Occupancy for a gymnasium and
recreational building located at 1493 County Road 106, near
Carbondale in Garfield County, Colorado under permit No. 5061
and,
WHEREAS, the following items have been determined to be
apparent defects or deficiencies at time of final inspection:
1. The installation of the siding has not been completed by the
General Contractor.
and,
WHEREAS, the Owner is willing to make certain assurances
and agreements relating to these identified code deficiencies and
on the basis of such assurances the County is willing to issue a
Certificate of Occupancy.
NOW, THEREFORE, it is agreed as follows:
A. The Owner agrees to do the following:
Deficiency No. 1.
ACTION REQUIRED OF THE OWNER:
The Owner will complete or cause the completion of the
installation of the exterior siding in compliance with
requirements of U.B.C. (Uniform Building Code) Section 2516
(g) .
FURTHER ACTION REQUIRED OF THE OWNER:
The Owner will complete all work described herein and
request inspection of all work by the Garfield County
Building Department. All work shall be completed in
compliance with the stated provisions of the codes currently
adopted by Garfield County in a manner that will enable
approval of all work. All work, inspections and approval of
work shall be completed within thirty (30) days of the date

of signing of this document.
B. In the event the Owner fails to perform as agreed on any
of the above, the County may revoke any permits which may

Subject: Observation of Excavation, Proposed CRMS Gymnasium Addition,
Carbondale, Colorado.
Gentlemen:
As requested, Hepworth - Pawlak Geotechnical, Inc. observed the excavation at the
subject site on April 13, 1994 to evaluate the soils exposed for foundation support. The
findings of our work and recommendations for the foundation design are presented in
this report. The work was done in accordance with our proposal for construction
observation and testing services to Colorado Rocky Mountain School, dated March 17,
1994.
The proposed gymnasium addition will be a steel and concrete form wall structure with
slab -on -grade floors. Foundations have been designed based on an allowable soil
bearing pressure of 2,000 psf, an equivalent fluid unit weight of 40 pcf and a sliding
coefficient of 0.35.
At the time of our visit to the site, the foundation excavation had been cut in 3 levels
from 1 to 3 feet below the adjacent ground surface. The soils exposed in the bottom of
the excavation consisted primarily of natural, dense, slightly silty sand and gravel. The
soils exposed at the center footing placed in the sewer line trench area were on
compacted fill. The soils exposed at the southwest footing area where it attaches to the
existing building consisted of silt and clay. The subgrade at three interior column pads
in the south portion of the excavation consisted of on -site fill. No free water was Praomle
observed in the excavation. We recommended to John Diemoz via telephone to remove 4/, i 3.9�
the fill under the interior column pads and the silt and clay under the southwest wall ✓ ro�kr
footing. The footings should bear on the lower undisturbed natural gravel soils. The e m plain(
center continuous footing can bear on the trench backfill where it crosses the sewer 6ou p,L GYM
trench. Density testing was not performed during baclffilling of the trench but we etall /b.
understand that it was backfilled with compacted on -site granular soils overlying 2 to 3
feet of flowable fill.
SV; Considering the conditions exposed in the excavation and the nature of the proposed
�dy .6 construction, spread footings placed on the undisturbed natural coarse granular soil
designed for an allowable soil bearing pressure of 2,000 psf should be adequate for
{1 support of the proposed gymnasium. Footings should be a minimum width of 16 inches pega JnP,D
for continuous walls and 2 feet for columns. Loose and disturbed soils encountered at ✓ H- /3 -91

extended down to the undisturbed natural coarse granular soils. Exterior footings
should be provided with adequate soil cover above their bearing elevations for frost
protection. Continuous foundation walls should be reinforced top and bottom to span
local anomalies such as by assuming an unsupported Length of at least 10 feet.
Foundation walls acting as retaining structures should be designed to resist a lateral
earth pressure based on an equivalent fluid unit weight of at least 40 pcf for on -site
granular soil backfill. A coefficient of 030 can be used to resist sliding for footings
placed on the dense gravel soils. A perimeter foundation drain should be provided to
- - - - - - - = -- prevent -tempara y buildup of hydrostatic - pressure - behind the-walls prevent wetting -
of the lower level. Structural fill placed within floor slab areas can consist of the on -site
soils or a suitable imported sand and gravel (such as 3/4" base course material)
compacted to at least 95 % of standard Proctor density at a moisture content near
optimum. Backfill placed around the structure should be compacted and the surface
graded to prevent ponding within at least 10 feet of the building.
The recommendations submitted in this letter are based on our observation of the soils
exposed within the foundation excavation and do not include subsurface exploration to
evaluate the subsurface conditions within the loaded depth of foundation influence. This
study is based on the assumption that soils beneath the footings have equal or better
support than those exposed. The risk of foundation movement may be greater than
indicated in this report because of possible variations in the subsurface conditions. In
order to reveal the nature and extent of variations in the subsurface conditions below the
excavation, drilling would be required. It is possible the data obtained by subsurface
exploration could change the recommendations contained in this letter.

Thanks for meeting with me today to review the carpet submitted by Diemoz
Construction for installation on the walls of the Colorado Rocky Mountain School
gymnasium. The carpet we reviewed was the Zodiac 30, 36 commercial carpet
manufactured by Aladdin Mills, Inc. A copy of the product test data is attached
for your files.
After a lengthy review of the manufacturer's data and test results of the
American Carpet Laboratories, Inc. you approved the carpet for installation on
the walls because the carpet has a NFPA Class I rating.

AGREEMENT REGARDING CERTIFICATE OP OCCUPANCY
WHEREAS, Colorado Rocky Mountain School(hereinafter "Owner ")
has applied for a Certificate of Occupancy for a gymnasium and
recreational building located at 1493 County Road 106, near
Carbondale in Garfield County, Colorado under permit No. 5061
and,
WHEREAS, the following items have been determined to be
apparent defects or deficiencies at time of final inspection:
1. The installation of the siding has not been completed by the
General Contractor.
. and,
WHEREAS, the Owner is willing to make certain assurances
and agreements relating to these identified code deficiencies and
on the basis of such assurances the County is willing to issue a
Certificate of Occupancy.
NOW, THEREFORE, it is agreed as follows:
A. The Owner agrees to do the following:
Deficiency No. 1.
ACTION REQUIRED OF THE OWNER:
The Owner will complete or cause the completion of the
installation of the exterior siding in compliance with
requirements of U.B.C. (Uniform Building Code) Section 2516
(g) .
FURTHER ACTION REQUIRED OF THE OWNER:
The Owner will complete all work described herein and
request inspection of all work by the Garfield County
Building Department. All work shall be completed in
compliance with the stated provisions of the codes currently
adopted by Garfield County in a manner that will enable
approval of all work. All work, inspections and approval of
work shall be completed within thirty (30) days of the date

8. In the event the Owner fails to perform as agreed on any
of the above, the County may revoke any permits which may
have been granted to the .Owner. The County may require the
