Dear Mr. Leach:
This letter is to give you a brief account of inspections on the shotcrete application
for the fire station at Missouri Heights.
An inspector was on site the following dates;
7/7/94
Inspector on site for scheduled shotcrete application, shotcrete crew was
unable to pump today due to equipment problems. Inspector went over test panel
requirements with the nozzle -man "Harry" . Test panels will be 2' X 2' X 4" in
dimension and will be shot and cured in the same manner as the walls themselves.
Inspector Hours; 1 hours
7/12/94
Inspector on site for scheduled shotcrete application but crew could not get
pump to run properly, at 12:00 they decided to re- schedule.
Inspector hours; 2 hours
7/18/94
Inspector was called as shotcrete was about to be placed, approximately 3
cubic yards were placed to "stiffen the wall" , the inspector told them to go ahead
with this small application and to shoot a test panel for later testing.
Inspector Hours; 1 hour
7/19/94
Inspector was on site to inspect application of 3 C.Y. of shotcrete on the
west wall exterior and the north wall interior. A test panel was shot for later
testing. The shotcrete was supplied by Roaring Fork Redi Mix. Shotcrete was
applied at right angles to the walls and adherence was uniform with minimum
rebound. The inspector was asked by Bill Gavett to take a look at the reinforcing
steel in the center and jamb columns at the vehicle entrance (south) wall. The
inspector noticed the #3 ties were on up to 24" centers and are shown on the plans
as being on 6" centers. This information was relayed to the contractor and Bill
Gavett. They said the engineer would be contacted to see if this spacing was
acceptable. If it is not acceptable the spacing will be redone.
Inspector Hours; 1.5 hours
7/20/94
Inspector was on site to inspect 2 C.Y. shotcrete application. The concrete
was originally ordered for the south wall but crew was correcting the tie spacing in
the columns and were not ready for the pour. Shotcrete was placed on the north
exterior wall. Pictures were taken showing the shotcrete and the corrected rebar
spacing.
Inspector Hours; 1 hours
7/27/94
Inspector on site to inspect steel being placed in top of north wall,
placement so far looks good but will need to be checked again before shotcrete is
placed. Photographs taken of steel in place. Shotcrete scheduled for Friday 7/29
or Monday 8 /1.
Inspector Hours; 1 hours.
8/3/ 94
Inspector on site to inspect steel and beam pockets on north wall. Beam
dimensions are correct and roof/ wall connection steel are in place. Shotcrete to
be placed for this wall today was postponed till 3:00 P.M. by concrete company,
then cancelled till tomorrow due to high wind.
Inspector Hours; 1 hours
8/4/94
Inspector on site for shotcrete placement at 6:20 A.M., 3 1/2 cubic yards
were placed. Application good, not much rebound. Crew completed placement by
9:00 A.M. Second load on site (2 1/2 cubic yards) was applied to exterior walls
which are near completion. All steel for this application was in place. Inspector
was informed by contractor that they would not return until Wednesday 8/4/94 and
would call for inspection when ready for placement.
Inspector Hours; 2.5 hours
8/11/94
Inspector on site for inspection of reinforcement and shotcrete located on
the 'exterior north wall and interior east wall (northeast corner). Approximately
1- 1/2 C.Y. were applied in these areas before crew moved to the south wall above
the door header. All plates and #4 bar were in place for this application. Beam
dimensions were checked and found to be correct. The remaining 2.5 C.Y. were
applied in this area to complete the shotcrete application for this project.
Inspector Hours; 2.0 hours
8/16/94
Inspector on site with Mr. Dan Downing, field manager for
CTL/Thompson; Materials engineers to take core samples from shotcrete test
panels taken July 18 and July 19, 1994. Cores were taken and tested in
accordance with UBC standard 26-10, 2621j. Test results are attached to this
letter.

Gentlemen:
CTL/Thompson, Inc. has completed compressive strength tests on cores obtained from shotcrete
panels cast on the subject site by the contractor. This testing was requested by Mr. Joe Bair with
Perfecta Services, Inc.
Compressive strength determinations were made in accordance with ASTM C -42 and C -39 and
U.B.C. Standard No. 26 -10. Test results are presented on Table 1.
